This is the online form for the Camper Application.

We will accept paper applications this year, if needed, but will transition to all digital next year. There will be some changes made, so please be on the lookout. We appreciate everyone’s patience and look forward to seeing everyone!

$300 Flat Fee Per Camper (includes all fees and camp t-shirt)

Camp Scholarship Application Due with Camper Application
     -$50 non-refundable deposit is required when submitting a Camp Scholarship Application to hold your camper’s spot!
- You will be notified by Camp HERO Board of Directors with the status of your camper’s scholarship
- Due to funding, the Camp HERO Board of Directors will review all scholarship applications and award scholarships based on need.
- We also encourage you to seek funding through local agencies ( i.e., Lions Clubs, Kawanis, etc.) that may be interested in sponsoring your child to come to camp! Please take a look at the Fundraising tab on our website.

***Multi-Child Discount***

This discount is for families who are sending more than one child to camp.  The multi-child discount cannot be applied to those who receive a camp scholarship.  Please indicate this on your camper's application.  Please submit all children's applications at the same time.  
                   first child is charged the full cost, then $15 off for additional children
                  

Registration MUST include:

*Payment
*Camp Scholarship Application & $50 Deposit
    (completed in its entirety if applicable)
*Medical form (Health History)
    (completed in its entirety)
*Photocopy of camper medical insurance   (front and back of card)
*Completed release forms: 
     medical release, photo/video release,
     field trip release form,
     and email/instant messenger release
*Types of hearing aid(s),
     cochlear implant device
     including model numbers
*Photocopy of drivers license
     or other photo identification of the
     individual responsible for picking up
     your child

Don't forget! Any application that is submitted incomplete will NOT be processed. Please refer to the above checklist to assure all items are submitted with your registration.
